This is why some people with severe emphysema develop a “barrel chest.” Emphysema falls in the broad category of chronic obstructive pulmonary disease (COPD).

What is COPD?
COPD is a progressive lung disease, and despite the sophistication of current medical treatments it is still considered to be “largely irreversible.” Indeed COPD is the fourth most common cause of death in the U.S.

Three lung disorders–namely, emphysema, chronic bronchitis and chronic asthma–can lead to COPD. Sometimes, doctors prefer to use the umbrella term COPD because a patient might have or may have had two or all three of the disorders. For example, chronic bronchitis or chronic asthma can develop into emphysema.

Emphysema is most often diagnosed in people who are in their fifties or sixties. In the initial stages, emphysema can be symptomless. The first sign of disease is when a person feels out of breath from doing normal, everyday activities. By the time the impairment seems serious enough to warrant medical investigation, much damage has already been done.

Emphysema carries a stigma because of its association with smoking: “You did it to yourself.” However, 15% of people with emphysema have never smoked. Nor do all smokers develop emphysema. Nevertheless, for anyone with COPD who still smokes, the single most helpful thing he or she can do is to stop smoking. Asthma is a respiratory disease that affects many children. Often it is associated with allergies, and most children outgrow asthma before they reach adulthood. However, some people suffer from asthma all their lives and may develop COPD. In some cases, adults who never had asthma as children develop it in later years.

Symptoms
COPD symptoms include chronic cough, chronic excessive mucus, acute bronchitis and breathlessness. Chronic cough and excessive mucus production often precede the development of airflow obstruction by many years. In the earlier stage, a person’s lung function may be impaired 20% to 30%, but he or she may not know it. Not all individuals with excessive coughing and mucus go on to develop COPD. However, in many cases, the progression of symptoms begins to affect the airflow to the extent that the individual experiences breathlessness even with mild exertion.

Unfortunately, Western culture doesn’t promote a high degree of breath awareness. Furthermore, people usually have a high tolerance for breathing discomfort, unless, of course, they are hyperventilating or unable to breathe. Therefore, the disease may go undiagnosed. People tend to cut down on activities that make them breathless rather than consult a doctor about their breathlessness.

When people do complain about breathlessness, they are often prescribed inhalers for symptom relief without undergoing a proper assessment of their lung function or getting a specific diagnosis. Anyone who experiences breathlessness should get a referral for a pulmonary function test (PFT).

Breathing muscles
In order to understand the breathing difficulty faced by people with COPD, let’s turn our attention to the primary and accessory breathing muscles. The diaphragm and intercostal muscles between the ribs provide the greatest force during inhalation and chest expansion. When relaxed, the diaphragm assumes a domelike shape, or more precisely, two domes, the larger on the left and smaller on the right, dividing the abdominal cavity from the chest cavity.

During inhalation, the dome-shaped diaphragm lowers and flattens. At the same time, the intercostals contract, lifting the ribs, which are at right angles to the spine, and increasing the entire volume of the chest cavity, thus allowing air to fill the lungs. With air chronically trapped in them, the lungs constantly press on the diaphragm. The diaphragm becomes progressively more rigid and flattened. Intercostals are shortened. Ribs “just stand there” and cannot provide additional force to make the inhalation effective. It is as if you have already inhaled without actually inhaling. Likewise, ribs don’t collapse sufficiently as required for effective exhalation.

With inadequate exhalation people with COPD often feel they are not getting enough air. Therefore, before they complete an exhalation, they start the next inhalation prematurely. As a result, their lungs become even more inflated over time. Air passages clogged with mucus and inflammation resist airflow and eventually collapse. The walls between the air sacs are destroyed. To compensate, lungs expand to grasp more air, which makes it even more difficult to effectively exhale.

As the diaphragm flattens and the rib cage loses its elasticity and thus its ability to lift, expand and collapse, the accessory breathing muscles are engaged. They include the pectoralis, sternocleidomastoid (SCM), scalene, trapezius, erector spinae and shoulder muscles, which must work harder to achieve inhalation. The abdominal muscles, too, must work harder to achieve exhalation. Strengthening the primary breathing muscles and restoring their ability to function may reduce the need for engagement of secondary muscles. For these reasons, one must learn to exhale more effectively. Long, slow exhalation not only helps to get the stale air out but also improves inhalation.

People with COPD should begin a yoga program with breathing retraining–exercises that can help strengthen their respiratory muscles, giving them some control over breathlessness. This will increase their readiness for asana practice as well as other forms of exercise.

Belly breathing

One should learn both techniques of diaphragm breathing: (1) abdominal breathing–pressing the belly in for exhalation and expanding the belly for inhalation and (2) side rib breathing–squeezing the side ribs with the hands while exhaling and releasing them while inhaling.

According to an individual’s level of fitness, he or she should learn forward bends, backbends, spinal extension and flexion, side bends and spinal twists. If there is a problem lying down, the poses should be done in either a standing or sitting position.

Some guidelines for practice of yoga poses

1. During the practice of postures, try to keep the exhalations longer than inhalations.

2. Some may prefer to do soft, slow, continuous breathing with pursed lips, which may provide greater stamina for exercising and also help control shortness of breath.

3. For those who are able, it is helpful to coordinate breathing with movement. As an example, in uttanasana (standing forward bend) one exhales while bending forward and inhales while coming back up, taking a longer time bending forward. Forward bends help one exhale more completely and strengthen the expiratory muscles.

4. Standing backbends such as the beginning of sun salutation are good for the chest muscles. But one should avoid excessive backbending or dropping the neck back. Some people can’t bring their arms to shoulder level or over the head. In such cases, arm and hand variations are necessary.

5. Mountain pose with arms stretched, hands clasped or hands behind the back in reverse namaste (for those who have such flexibility) helps with axial extension and chest mobility.

6. Warrior I is good for strengthening the breathing muscles employed during inhalation and exhalation.

7. Standing side bends such as triangle are particularly good for strength and flexibility of the rib cage and for improving the capacity for side rib diaphragm breathing. If one experiences neck tension or breathing difficulty, one should look down rather than up at the extended hand in triangle pose. If one has difficulty standing and balancing in the triangle position, sitting side bends are a good alternative.

8. Cat pose can help with exhaling and inhaling more effectively. Extending the legs alternately in the cat pose, sometimes referred to as “dynamic cat pose,” may be done to strengthen the hips and legs. For me cat pose is highly restorative. It helps me optimize the inhalation-exhalation ratio. Depending on my breathing status at the time, I can work more on exhalation or inhalation.

9. Combining the cat pose with the kneeling pose vajrasana (thunderbolt) in a flowing (vinyasa) fashion–from cat pose, one brings the buttocks to the heels while exhaling, then rises up to a kneeling position with arms overhead while inhaling I ADDED THIS USING GARY KRAFTSOW’S BOOK–I HOPE IT’S RIGHT–can be very helpful for strengthening the respiratory muscles that are engaged during exhalation.

When people with COPD become proficient at yoga postures, they will also be able to include in their routine such activities as walking daily for 30 to 60 minutes, occasionally swimming and lifting light weights once or twice a week.
